CA213 Shortage Area Siskiyou CountyProject SummaryDr. Amy FousekI am a past VMLRP recipient for CA213 and have been able to establish a successful large animal mobile practice in the area over the last five years. I am requesting funding to help my large animal mobile practice grow by adding an associate and starting a haul-in facility. I will be purchasing a mobile chute, a vet box, ultrasound unit, blood machines, veterinary computer software and a microscope to improve the quality of veterinary medicine I am practicing, keep staff safe and reach more clients/animals in the shortage area I currently practice in. The equipment and salary support for a new associate will be encouraging to a new graduate to stay in a rural area and work on food animals relieving a veterinary shortage situation that has only gotten worse since I arrived in 2016.

Goals / Objectives
I started my mobile large animal practice in 2016 when I moved to Siskiyou County (Shortage Area CA213). I moved here to address the veterinary shortage situation.My business has grown tremendously over the last five years and is very busy for a single practitioner. The food animal/production species I currently work on are beef cattle, dairy cattle, small ruminants, camelids, swine, backyard poultry and honey bees. Food animal composes 50% of my practice. Since I moved to this shortage area we actually have fewer veterinarians offering food animal services. One mixed animal practice closed their doors when the owners couldn't find new associates and elected to retire. Meanwhile two practices in the county stopped working on food animals as the veterinarians are now in their 80's and trying to completely retire in the near future. We are in a dire situation! I am working as many hours and days as possible. I am also on call the majority of nights and weekends and do not have another veterinarian to share call with. The county I cover is vast and I am easily driving an hour in each direction to field calls. I routinely travel 45 miles to Etna, CA where I have a small facility for equine clients.In 2017 the USDA estimated 47,000 head of cattle in our county which is higher than our population of 44,000 people. Siskiyou County is 6,347 square miles and I try to offer my services to the entire county. I would like to offer regular veterinary care for an area two hours away in Happy Camp, CA but struggle to be that far from the rest of my clients. When I have an associate, I plan to grow clientele in Happy Camp/Siead Valley (90 miles west), Dorris (58 miles east), and Forks of Salmon (84 miles southwest).I have over 1000 clients in my records and see new clients every week. I average 5 to 10 calls per day.My first goal is to expand my large animal mobile practiceto reach more clients andanimals specifically bovine, small ruminants and swinein the shortage area CA 213. A second goal, that goes hand in hand, is to make my practicemoreattractive for a new associate to workin a rural area and work on food animals relieving this veterinary shortage situation.Objectives:Build a haul-in facility and office by 2022.Hire a veterinary associate and provide a competitive salary.Upgrade equipment by purchasing an ultrasound, microscope and in house blood machines by 2022.Set up a second mobile unit for the assoicate by purchasing a new veterinary box.Use veterinary software to go to digital record keeping, schedule and inventory.Send new graduate to continuing education classes.Offer veterinary care to additional areas within the area.Additional goals I have are to improve the quality of veterinary medicine I am practicing.Objectives:Attend continuing education for ultrasound training and bovine and small ruminant medicine.Use phase contrast microscope to improve breeding soundness exams.Offer fetal sexing and improve fetal staging with Ibex ultrasound.Offer better diagnostics with the ability to get timely results on CBC/Chem tests with in house blood machines.My final goal is to improve staff safety in the field and to be used in my future haul in facility.Obejective:Purchase hydraulic mobile chute.
Project Methods
EffortsThe method I will use to complete my project is to first purchase the equipment I need, complete construction of my haul in facility andhire an associate veterinarian.For the community outreach side of my project my methods will be:1) I will continue to be the leader for the veterinary science 4-H program which includes formal classroom, laboratory hands on activities and practicum experience.2) I will plan yearly producer meetings for client education3) I will contact local veterinary schools and set up externships for veterinary students which provides practicum experience fourth year veterinary students.To expand my service area to Happy Camp I will contact the local ranchers and community hall personnel to set up my monthly visits/mobile clinics.EvaluationI will evaluate my success by comparing my active client database andmy food animal related incomeeach year.

Target Audience:Rural clientele, food animal clientele, 4-Hers, high school students, undergraduate pre-vet students, veterinary students Changes/Problems:As I reported in previous years, it took almost 3 years for my county building department to give me the permit for building ofmy brick and mortar practice. Theextended wait time caused a dominoaffect in my plans and goals for my business. I delayed hiring an associate because we did not have an office to work out. I have delayed purchasing the chute until I was able to build a corral system at my new building. Because of the delays, I requested a year extension to spend the money awarded to me in the grant and reach my goals. My request was accepted. The good news is that the my clinic is built!!I have offered an associate position toan excellent candidate with ties to the rural community we live in and plan to be on track with the last purchases I budgeted for in the veterinary service grant program. What opportunities for training and professional development has the project provided?Purchasing the new ultrasound encouraged me to learn fetal sexing. I was able to take online courses to learn this skill. How have the results been disseminated to communities of interest?I always update my clients about the grant and give credit to the VSGP for recent purchases such as the veterinary box. I have made announcements at producer meetings, cattlemen's events and 4-H meetings. What do you plan to do during the next reporting period to accomplish the goals?I plan to purchase the cattle chute and build safe cattle handling facilities at my brick and mortar practice. I plan to mentor a veterinary student/future associate during her final year of school and externships. I will be providing her with an employment contractin the near future.

Impacts
What was accomplished under these goals? In the last year I have purchased the veterinary software and implemented it in the practice. I use it to schedule appointments, track inventory, invoice clients and improve my medical records. Recently I was able to integrate the blood machines with my practice management software and results are recorded directly into patient records. I purchased a chasis mounted veterinary box and installed it on a new truck. The new veterinary box has beena huge upgrade. It makes my jobeasier and my supplies and instruments are stored in a clean,organized fashion. The refrigerator, freezer, hot water, inverter for power supply, work surface and lock box for drugs have all been life changing! I have retained my older veterinary truck and box for my future associate. I finally completed the building of a veterinary office! I am utilizing the brick and mortar addition to my practice to offer haul-in appointments forsmall ruminants, equine, poultry and small animal. It has provided more space for my equipment and improved organization. I am now ready to hire an associate which leads me to another major achievement. I have offered theassoicate position to a new graduate interested in a mixed animal career. She is also interested in living in the area as her parents live near by. I have continued to learn the skill of fetal sexing with the new ultrasound. It has definitely improved my images and made fetal staging more accurate. The ultrasound has been valuable in diagnosis and treatment of various conditiions in cattle and small ruminants i.e. urolithiasis in goats. I continue to offer services to the most rural, veterinary shortage areas (Happy Camp) on a quarterly basis.

    Target Audience:In the last year I have reached 4-H'ers, high school students, Davis Veterinary students, ranchers and clients of Siskiyou County. Changes/Problems:One problem I have encountered was not realizing all the challenges that would exist in constructing a brick and mortar facility i.e. how long it would take to get a permit for my remodel plans, how slow construction would be and the high costs of building materials. The next problem I have encountered is finding a veterinary associate. New grads are in high demand and have multiple offers upon graduation. I was not planning on the delay of purchasing items (chassie mounted veterinary box had a 2 year wait time) or the increase of price of items over the last2years. The items I budgeted for have gone up in price. What opportunities for training and professional development has the project provided?The project and equipment purchased through the project have helped with training of staff and students. It has also provided opportunities for 4-H youth i.e. visualize what I am seeing on the microscope using the wifi camera. How have the results been disseminated to communities of interest?I have used social media and posters to reach the underserved communities. I have announced the project and plans at the cattle producer meetings. What do you plan to do during the next reporting period to accomplish the goals?In the next and final year of the project I plan to complete the haul in facility and advertise for an open associate position with veterinary schools. I plan to become proficient with new ultrasound bytaking an ultrasound course to learn fetal sexing and improve my skills at fetal staging. I plan to continue offering quarterly ranch calls and vaccination clinics to underserved areas of Siskiyou county. I plan to implement the new practice management software to better track inventory, scheduling, medical records andreports to track revenue. I plan to complete the purchases of the veterinary box and chute.

    Impacts
    What was accomplished under these goals? 1. The haul in facilty was granted a permit by the county and is over 50% completed. 2. I invited a senior veterinary student to extern at my practice and was able to offer her a position after she graduates. 3. I have purchased the microscope and improved my diagnostic capabilities especially in regards to bovine breeding soundness exams. The wifi capabilities has allowed me to improve client education. I have also used it for education with the 4-H youth. The ultrasound is on order. The blood machines have been in use for over a year and have been helpful with critical cases. 4. I have ordered a vet box 5. I have attended software demo meetings to chose software that will fit my practice's needs and am currently deciding which program fits my business needs. 6. I have not hired a new graduate yet 7. I have successfully offered regular veterinaryservices to an underserved area (Happy Camp/Seiad Valley) on a quarterly basis. I have successfully improved the quality of the veterinary medicine I am practicing with the new microscope and blood machines. Addressing my final goal of improving safety, I have a concrete slab set to install a new chute at the haul in facility. I have investigated and researchedseveral companies to decide on which chute would fit our needs the best.

      Target Audience:Clients in need of emergency care, food animal clients of Siskiyou county, and clients in Happy Camp and Seiad Valley who have minimal access to veterinary services Changes/Problems:My largest struggle is getting the building permit to start construction on the remodel of an existing building. Due to COVID our county planning department was back logged and took 9 months to look at my permit. I was then required to make changes to the engineered plans and resubmit. The plans will be resubmitted Decemeber2022. Currently, I am running my business out of my house and am desperate to get the haul in facilities and office up and going. I haven't bought all of the equipment in my budget yet as I don't have a place to store it. My plan to hire an associate over the last year was not successful. The interested party did not pass the California board exam. She obtained a Oregon license and employment 1 hour north of us. Manufacturing of goods has suffered many delays in the last couple years. My first contact for purchasing a chassis mounted vet box closed his shop. I am on a 2 year wait list for manufacturing the vet box in my grant proposal. I will continue to work away at my goals over the next 2 years. What opportunities for training and professional development has the project provided? Nothing Reported How have the results been disseminated to communities of interest?I announced the award of this grant at the cattle producers meeting that I hosted. What do you plan to do during the next reporting period to accomplish the goals?During the next reporting period, I plan to start construction on the corrals and prepare for the addition of the hydraulic chute. I plan to have the building permit to start remodeling my existing building and start construction. I plan to make the purchases of the ultrasound and microscope. I also plan to purchase the veterinary software program in January 2023in order to start the new year with the program. Once I've purchased the new ultrasound I plan to attend a workshop to improve my ability to use it and offer additional services such as fetal sexing.

      What was accomplished under these goals? In reference to the goals listed in my application, I have worked to expand my large animal mobile practice. I have purchased in house blood machines and have them in use reaching my objective of providing better diagnostics. I have ordered a veterinary box to create a second mobile unit. I was able to demo an ultrasound unit to help with the decision of which one to purchase but found the battery life inhibiting for chute side use. Many of the locations I practice in do not have electricity at the corrals. I have scheduled large animal calls and a small animal Rabies clinic in the remote town of Happy Camp, CA. The day spent servicing this veterinary shortage area of Seiad and Happy Camp was appreciated by the residents. The Rabies clinic was well attended with 52 Rabies vaccinations given in 2 hours. This area is recovering from a devastating wildfirethat burned 60,000 acres between Seiad Valley andKlamath River. Itdestroyed 185 structures and claimed the life of 4 people and uncountable animals.I treated burned cows, a calf and a bull from a herd that were victims of the McKinney Fire. Small steps toward building the haul in facilities were achieved as I am still waiting a countybuilding permit for my remodel project.
