Reinvestment Partners (RP) is seeking $500,000 over three years for RP Rx-VA, a Produce Prescription Program, in partnership with the Durham Veterans Affairs (VA) Health System in North Carolina. RP Rx-VA provides low-income Durham VA patients who receive SNAP benefits with a monthly prescription to purchase fruits and vegetables from Food Lion, major grocery retailer in the Southeast. The program is unique because it focuses on veterans, partnering with the Durham VA to refer participants and provide wraparound services to clients with nutritional needs.

Project Methods
Eligible produce prescription participants are veterans with nutrition or diet-related health concerns that are receiving care from the Durham VA Health System. The veteran must also receive SNAP benefits. The VA case managers will screen patients with nutrition-related health issues about their access to healthy food. The providers will ask if the client receives SNAP benefits, inform them about the RP Rx-VA Produce Prescription Program, and, with the client's permission, enroll them in the program. The enroller can be a case manager or a health provider, but we anticipate health care case managers handling most of the enrollment.Durham VA enrollers will use a web portal created by RP to enroll veterans in the program. During this process, enrollers enroll participants electronically into RP Rx-VA and include the participant's Food Lion MVP card number as part of that enrollment process. If the participant does not have a Food Lion MVP card, the enroller can help them get a card through Food Lion's website, which will generate an MVP number for the participant. RP's enrollment website connects with Food Lion through a secure FTP site, which automatically activates the participant's Food Lion MVP card and designates them as a RP Rx-VA participant.Participants load $40 in produce prescription funds onto their loyalty card by using their EBT card to buy SNAP eligible food at a participating Food Lion. The RP Rx-VA produce prescription funds can then be used to purchase approved fruits and vegetables using the Food Lion loyalty card. The eligible fruits and vegetables are WIC-approved fresh, frozen, and canned fruits and vegetables with no added sugar, salt, or fat.

The Durham VA's accredidation board used information from the Eat Well program to satisfy health equity accredidation requirements with the Joint Commission. Impacts
What was accomplished under these goals? RP distributed $439,000to veteran participants during the grant period. With these funds, low-income veterans experienced reduced barriers to healthy eating and increased their purchase and consumption of fruits and vegetables. RP's program used technology to distribute funds quickly and efficiently to participants. Participants were able to purchase the eligible items of their choice at Food Lion stores that were most convenient for them. RP executed a contract with Durham VA researchers to evaluate the outcomes of the program. The findings are forthcoming.

Changes/Problems:We encountered challenges with executing a legal agreement with the VA, which has been resolved. We continue to seek higher level agreements to improve the capacity of our program to serve more veterans. Similarly, we have had challenges identifying a research partner. We continue to seek out this partnership. What opportunities for training and professional development has the project provided? Nothing Reported What do you plan to do during the next reporting period to accomplish the goals?During the next reporting period, RP seeks to imporve our outreach and enrollment strategy. We also seek to establish a research partnership with evaluators within the Durham VA.

Impacts
What was accomplished under these goals? During this reporting period, RP accomplished the following: Executed a legal agreement with the Durham VA to implement the program Enrolled 539veterans Distributed $66,404 foreligible fruits and vegetables. This reduced the financial barrier to healthy eating for participating veterans. Preliminary research has found that these benefits double participants' intake of fruits and vegetables compared to before they were enrolled. Used technology to quickly and efficiently distribute nutrition assistance to enrolled veterans.

    Target Audience: The RP Rx-VA program will be implemented through partnerships with the Durham VA health system, which serves 27 counties in central and eastern North Carolina. Changes/Problems:It has been challenging to navigate the bureaucracy and slow-moving processes at the Durham VA. As outsiders, it's difficult to find a champion within a complex system of providers and administrators. It took more than one year to execute a Memorandum of Understanding. Now that it is signed, we are hopeful we can ramp up enrollment and connect veterans in need with fruits and vegetables. In particular, we are working with an endocrinologist-led team in Greenville, NC right now.
