Progress 05/01/20 to 04/30/21
Outputs Target Audience:Parents of preschool children Parents of school-age children School-age children Undergraduate students, graduate students, post-doctoral associates, dietetic interns Changes/Problems:The COVID pandemic has stalled our progress, especially with recruiting participants and collecting data. We are poised to resume these activities as soon as guidance from authoritative sources (e.g., our universities, CDC) deem it safe to do so. What opportunities for training and professional development has the project provided? This project has formally and informally trained undergraduate and graduate students to develop obesity prevention intervention materials and conduct focus groups and surveys; created obesity prevention intervention materials; created and implemented training materials for instructors and facilitators, monitored data collection activities; recruited and retained participants, and managed and analyzed data. Additionally, we have provided online virtual training in childhood obesity prevention for childcare workers nationwide. How have the results been disseminated to communities of interest? Results have been disseminated via presentations at professional meetings and peer-reviewed journal articles. What do you plan to do during the next reporting period to accomplish the goals? The achieve our goals for the next year when COVID-related circumstances are deemed safe to proceed, we plan to continue to recruit preschools to participate in the Goal 1 dissemination study. In addition, we plan to finalize Goal 2 Part D (formative/developmental evaluation) and Part E (finalize website and delivery materials for SNAP-Ed participants), and Goal 3 to implement the randomized controlled trial. We also plan to continue work on all parts of Goal 4.
Impacts What was accomplished under these goals?
Goal 1 The dissemination feasibility study is underway this year, but has stalled due to COVID-related closing of preschool and childcare. Part A-The re-designed and re-programmed website to be fully responsive to variable screen sizes (e.g., different sizes of screen on phones, tablet, and computer monitor) was completed and has been maintained to current standards. Part B-Findings from preschool director interviews were used to create parent recruitment materials, resources, and document dissemination activities. Formative testing is complete. Findings were presented at the American Society for Nutrition annual meeting in June 2018 and are being used to implement the dissemination study. Part C-The brief trainings for preschool/daycare staff on childhood obesity prevention and benefits of disseminating the HomeStyles program to parents have been designed with implementation ongoing earlier in this study year, but now stalled due to COVID-related closings of preschools can childcare agencies. Goal 2 The creation of the second segment of HomeStyles tailored to families with children 6 to 11 years is nearly complete; progress was stalled due to COVID-related circumstances. Part A-The HomeStyles Segment 2 materials are aligned with obesity prevention recommendations from the IOM, CDC, White House Task Force, Healthy People 2020, Dietary Guidelines, and SNAP-Ed and EFNEP guidance. Constant surveillance was conducted to identified new and/or revised recommendations for inclusion in materials (e.g., Dietary Guidelines 2020-2025 and Healthy People 2030). Part B-Throughout development of these materials, parents in Florida, New Jersey, and West Virginia have been involved by participating in focus groups and cognitive interviews. Focus groups were conducted with 194 children and 185 parents. Cognitive interviews have been conducted with 78 parents. Faculty and graduate students were trained to content analyze focus group data; findings were presented at the American Society for Nutrition in June 2018 or Academy of Nutrition and Dietetics in October 2018 or October 2019. Focus group findings have been published in peer-reviewed journals. Cognitive interviews with parents related to the intervention guide content have been completed with English speaking parents. Guides have been translated and cognitive interviews of content were completed with Spanish speaking parents. Materials for children have been written and the content cognitive tested. Design cognitive interviews of children's materials were completed. Part C-Methods for assessing the degree to which home environments and weight-related lifestyle practices to prevent excessive weight gain in middle childhood have continued and have been field tested. The assessment instruments have been identified/modified/created, undergone content validity review, refined, undergone cognitive testing, pre-tested, pilot tested, field tested, psychometrically analyzed, and refined. Data dictionaries and protocol manuals have been refined and finalized. The finalized assessment instruments have been translation to Spanish and an online survey created. Part D- Formative/developmental evaluation is underway, but has been stalled due to COVID-related circumstances. Part E-Plans for comparing the 2 delivery modes are nearing finalization, but has been stalled due to COVID-related circumstances. Web delivery of materials is nearly completion, but has been stalled due to COVID-related circumstances. The online randomized controlled trial is registered with ClinicalTrials.gov (ClinicalTrials.gov Identifier: NCT04802291) and protocol paper is nearing completion for submission to a peer-reviewed journal. Goal 3 Work on this goal but has been stalled due to COVID-related circumstances. The hope is to begin the randomized controlled trial in mid-2021. Goal 4 Part A-The 1-credit childhood obesity course developed and taught in Year 1 and refined and offered again in Year 2 was further refined into self-instructional modules (topics include Quantitative data collection and analysis, Systematic Literature Review, Abstract Writing, nutrition legislation and policy, role of land grant institutions, obesity prevention programs, and skills for reporting research findings including creation of an infographic reporting results, Quantitative Data Collection and analysis, research protocol development, Research Paper writing and Presentation). Part B-A total of 6 undergraduate students, 10 graduate students, 4 dietetic interns, and 2 post-doctoral associates have participated in mentored practicums. Part C-Self-instruction modules were created and field-tested with 2 graduate students and 3 dietetic interns. Modules are being utilized in undergraduate and graduate instruction Year 2 forward. Plans to create free-standing, self-instructional, continuing education modules for nutrition and health professionals are underway.

Additionally, we have provided online training in childhood obesity prevention for child care workers nationwide. How have the results been disseminated to communities of interest? Results has been disseminated via presentations at professional meetings and peer-reviewed journal articles. What do you plan to do during the next reporting period to accomplish the goals? The achieve our goals for the next year, we plan to continue to recruit preschools to participate in the Goal 1 dissemination study. In addition, we plan to finalize Parts A to C of Goal 2 and implement the process and outcome/impact evaluation component of Part D and effectiveness comparison of Part E. We also plan to continue work on all parts of Goal 4.
Impacts What was accomplished under these goals?
Goal 1 The dissemination feasibility study is underway. Part A- The re-design and re-programming of the website to be fully responsive to variable screen sizes (e.g., different sizes of screen on phones, tablet, and computer monitor) was completed. Part B-Findings from preschool director interviews were used to create parent recruitment materials, resources, and document dissemination activities. Formative testing is complete. Findings were presented at the American Society for Nutrition annual meeting in June 2018. Part C-The brief trainings for preschool/daycare staff on childhood obesity prevention and benefits of disseminating the HomeStyles program to parents have been designed with implementation ongoing. Goal 2 The creation of the second segment of HomeStyles tailored to families with children 6 to 11 years is in progress. Part A-The HomeStyles Segment 2 materials are aligned with obesity prevention recommendations from the IOM, CDC, White House Task Force, Healthy People 2020, Dietary Guidelines, and SNAP-Ed and EFNEP guidance. Constant surveillance is conducted to identified new and/or revised recommendations for inclusion in materials. Part B-Throughout development of these materials, parents in Florida, New Jersey, and West Virginia have been involved by participating in focus groups and cognitive interviews. Focus groups were conducted with 195 children and 158 parents. Cognitive interviews have been conducted with 78 parents. Faculty and graduate students were trained to content analyze focus group data; findings were presented at the American Society for Nutrition in June 2018 or Academy of Nutrition and Dietetics in October 2018. Focus group findings have been published or are in review in peer-reviewed journals. Cognitive interviews with parents related to the intervention guide content have been completed with English speaking parents. Guides are being translated and cognitive interviews of content are planned with Spanish speaking parents. Part C-Methods for assessing the degree to which home environments and weight-related lifestyle practices to prevent excessive weight gain in middle childhood have continued and are being prepared for the pilot test. Part D- Formative/developmental evaluation is underway. Part E-Plans for comparing the 2 delivery modes are nearing finalization. Goal 3 No work on this goal is scheduled until the randomized controlled trial being developed in Goal 2 is near completion. Goal 4 Part A- The 1-credit childhood obesity course developed in Year 1 was refined and offered again in Year 2. It was taught synchronously online (via Zoom) at 3 land-grant institutions (i.e., Rutgers, University of Florida, West Virginia University) in fall 2018. Topics included Dietary Guidelines for Americans, nutrition legislation and policy, role of land grant institutions, obesity prevention programs, and skills for reporting research findings including creation of an infographic reporting results and skills for effectively educating legislators about nutrition programs. Part B-A total of 6 undergraduate students, 10 graduate students, 4 dietetic interns, and 2 post-doctoral associates have participated in mentored practicums. Part C-In addition to those created in Year 1, an array of teaching modules were created and used in the formal graduate course and to train students participating in mentored practicums.

What opportunities for training and professional development has the project provided?Training or Professional Development: formally and informally trained undergraduate students and graduate students to develop obesity prevention intervention materials and conduct focus groups and surveys; created obesity prevention intervention materials; created and implemented training materials for instructors and facilitators; monitored data collection activities; recruited and retained participants; managed and analyzed data. Provided online training in childhood obesity prevention for child care workers nationwide. How have the results been disseminated to communities of interest?Written reports of Preschool Director interview results were dissemenated to participants. Results have been disseminated via presentations at professional meetings. What do you plan to do during the next reporting period to accomplish the goals? To achieve our goals for the next year, we plan to complete all parts of Goal 1. In addition, we plan to complete Parts A to C of Goal 2 and begin Parts D and E. We also plan to continue work on all parts of Goal 4.
Impacts What was accomplished under these goals?
Goal 1 Part A-The re-design and re-programming of the current website to be fully responsive to variable screen sizes (e.g., different sizes of screen on phones, tablet, and computer monitor) is underway. Re-design is nearing completion with re-programming in early stages; anticipated completion in late spring/early summer. Part B-Trained researchers conducted 2 rounds of interviews with preschool directors (n=9) from 3 states (FL, NJ, WV). Round 1 of interviews gathered insight into how best to build interest in the HomeStyles dissemination feasibility study among preschools and parents. Round 2 of interviews gathered feedback on the summary of round 1 interviews and provided further insight into disseminating the HC through preschools. Findings are being used to create parent recruitment materials, resources, and document dissemination activities, with formative testing scheduled for late spring. Findings have been submitted for presentation at the American Society for Nutrition annual meeting. Part C-Training materials are in development, with the project overview and background modules completed. Additional training materials focusing on the specifics of implementing the dissemination study are in the planning phase. Goal 2 Part A-All guidance documents have been collected, reviewed, key concepts abstracted. Constant surveillance underway for new and/or revised recommendations. Part B-Focus group protocols were developed to address 12 obesity-related topics with parents and 11 topics with children aged 6 to 11 years. Focus group moderators and note-takers in 3 states (FL, WV, NJ) were trained to expertly conduct focus groups with parents and with children. Child-only focus groups where conducted with a total of 194 children aged 6 to 11 years and parent-only focus groups were conducted with 185 English-speaking and XX Spanish-speaking parents of children. Focus group participants were nearly equally distributed across FL, WV, and NJ. Faculty and graduate students were trained to content analyze focus group data to identify trends and themes. Findings have been submitted for presentation at the American Society for Nutrition annual meeting or Academy of Nutrition and Dietetics annual meeting. Part C- A compendium of valid, reliable, efficient assessments has been assembled and enhanced regularly. Preliminary evaluation of these instruments' suitability to the current study has occurred and needs identified. Activities to adapt extant measurements and/or create de novo measurements are underway and will continue through the first have of Year 2. Early analyses have been submitted for presentation at the Academy of Nutrition and Dietetics annual meeting. Part D- Work toward this objective is scheduled to begin in late 2018. Goal 3: no work scheduled for Year 1 of project Goal 4 Part A-A 1-credit childhood obesity prevention (COP) graduate course was taught synchronously online (Canvas) at 3 land-grant institutions (i.e., Rutgers University, University of Florida, and West Virginia University) in fall 2017. Topics designed to build COP expertise of the 14 enrolled graduate students and dietetic interns and 11 auditing graduate students included: HomeStyles project as a case study, systematic literature reviews related to COP topics, behavior change theory and motivational interviewing application to weight-related behaviors, mission of land grant institutions, Cooperative Extension programs vis-à-vis weight-related behaviors, and skills for reporting research findings at professional meetings. Findings have been submitted for presentation at the American Society for Nutrition annual meeting. Part B-A total of 12 undergraduate students, 10 graduate students, 2 post-doctoral associates, and 7 dietetic interns have participated in mentored practicums. Part C-An array of teaching modules were created and used in the formal graduate course and to train student participating in mentored practicums
