Non Technical Summary
This project will support the implementation of a multilevel, multisector, community-basededucational program with the overall goal of improving chronic disease self-management amongrural residents in Florida. We will develop, implement, and evaluate the effectiveness of theprogram in two phases. During phase 1, we will utilize a community-engaged approach to developa tailored multilevel intervention focused on self-management strategies to improve healthoutcomes. In Phase 2, we will implement and evaluate the effectiveness of the program among ruraladults in the three-targeted counties. We will enroll up to eight cohorts of approximately 10participants per county (total projected N=240). At the conclusion of this project, at least 75% ofparticipants will report increased participation in at least one health screening behavior, i.e., bloodpressure and/or blood glucose monitoring, monitoring their weight, and annual physical exams withtheir health care providers, at least 75% of participants will report increased intent to follow theirmedical treatment plan or ask their medical provider for additional information related to managinga cardiometabolic condition, at least 75% of participants will report increased knowledge of hownutrition impacts cardiometabolic conditions and intent to change dietary habits, at least 75% ofparticipants will report increased knowledge of the types of exercise recommended and intent toincrease physical activity levels, and at least 75% of participants will report increased self-efficacyrelated to making healthy lifestyle changes. This project will produce a new educational programthat can be scaled across Florida and utilized throughout the Cooperative Extension System.

Goals / Objectives
Our goal is to implement a multilevel community-based education and action program with the overall goal of providing health education and behavioral skills to recognize and address the progression and management of multiple chronic conditions (MCCs) in rural adults.Objectives?1. Develop a tailored educational program for rural residents living with chronic conditions, especially MCCs, within 15 months of project initiation.2. At least 75% of participants will report increased participation in at least one health screening behavior, i.e., blood pressure and/or blood glucose monitoring, monitoring their weight, and annual physical exams with their health care providers.3. At least 75% of participants will report increased intent to follow their medical treatment plan or ask their medical provider for additional information related to managing a cardiometabolic condition.4. At least 75% of participants will report increased knowledge of how nutrition impacts cardiometabolic conditions and at least 1 change they can make toward developing healthy eating habits5. At least 75% of participants will report increased intent to engage in at least 2 changes in dietary or eating habits within 1 month of completing the program.6. At least 75% of participants will report increased knowledge of the types of exercise recommended and ways to engage in physical activity.7. At least 75% of participants will report increased intent to engage in increased physical activity within 1 month of completing the program.8. At least 75% of participants will report high confidence in being able to make at least 2 healthy lifestyle behavioral changes within 1 month after completing the program.
Project Methods
During the 3-year project period, we will utilize a community-engaged approach to develop, implement, and evaluate the impact of the managing multiple chronic conditions (MCC)program in three rural communities. This program will be guided by the Cooperative Extension Framework for Health Equity and Wellbeing and utilize the Health Belief and Socioecological Models as a basis for program design and implementation. For the proposed project we will: 1) utilize quantitative and qualitative data from our target population to develop a community-based health education program that focuses on self-management strategies to improve health outcomes, 2) engage our priority population and their social support systems in programming to promote healthy lifestyle changes and other strategies to effectively manage MCCs and/or mitigate the impacts of living with MCCs.We will enroll up to eight cohorts of approximately 10 participants per county (Total projected N=240).We will monitor and evaluate our progressto determinethe extent of adoption, reach, implementation, and effectiveness in the Rural HEALTH 3 Project. The focus is on evaluating assessment data; county coordination of intervention implementation; and the effectiveness of the program components in changing short-term outcomes targeted by the program.Design: The evaluation is a retrospective pre/post-test evaluation design with no comparison group.Data collection: A variety of data collection tools are referenced throughout the evaluation plan.Formative evaluation and tools: This project will evaluate the efficacy of program delivery using a formative evaluation framework. We will leverage a mixed methods approach to understand the participants' experience and satisfaction with the program, in order to promote data-driven remedial actions and inform best practices for program transferability. We engaged in semi-structured interviews with the priority audience to conduct a needs assessment for the project.
