Outputs Target Audience:The state of Alabama has an SBIR grant matching program of up to $250,000.We submitted an application to continue this work toward commercialization at the end of July and expect to be notified in the next 2-3 months. We are pursuing several National Institute of Health (NIH) grants to continue the development of the CDSS during this SBIR.We are partnering with the University of Alabama and DCH Hospital for these grants. For the Sleep Navigator, we are working with a local clinic to do continued testing and research.We are also pursuing NIH grants and research grants from pharmaceutical companies. We are continuing to have discussions with two local hospitals about the possible utilization of TELECARE-ICU once it has received FDA approval.They had expressed strong interest and wanted to be kept up-to-date as we move forward.We have also had initial discussion with Medtronic, Allscripts, and Pheiser about possible partnerships.They too had expressed interest and wanted to be kept up-to-date as TELECARE-ICU is developed. What opportunities for training and professional development has the project provided?For the COVID Sleep Health Navigator we have developed content for educating staff throughout the hospital about sleep/ breathing disorders and the impact of long-term COVID on sleep disorders How have the results been disseminated to communities of interest?It has been distributed to the clinical staff involved in sleep studies. We plan to publish our content in the appropriate sleep related journals. What do you plan to do during the next reporting period to accomplish the goals?Finalize the demo Process an additional data set we have just received and updated the Clinical Decision Support Algorithm as needed Complete the final report

Outputs Target Audience:We are expanding the R&D for the TELECARE ICU solution to expand its capabilities to enhance our solution as described below: We are developing Large language models (LLMs) to work with the TELECARE-ICU system to provide access to additional information in a more timely manner. This begins by processing medical literature, clinical guidelines, and patient data, so the LLMs can offer personalized recommendations, answer complex queries, and assist in diagnosis and treatment planning along with the results of the TELECARE-ICU system. Once complete, we expect these models can help healthcare providers in rural areas to stay updated with the latest medical advancements, identify potential treatment options, and improve patient outcomes. Additionally, LLMs can be used to develop natural language interfaces, allowing clinicians to interact with the TELECARE-ICU system more intuitively and efficiently.Future capabilities could also include automated language translation. We continue to pursue grant opportunities from NIH and other agencies to finalize the development and testing so that we can receive FDA approval. Changes/Problems:

How have the results been disseminated to communities of interest?In support of Magic Medical Solutions' USDA Phase II SBIR COVID-19, TELE Case Assessment Response Engine ICU (TELECARE-ICU), we have launched a web site (https://covidsomnia.org/) to serve as a central resource for COVID-Somnia researchers throughout the United States and around the world.The site includes an exhaustive list of publications on COVID-Somnia.along with a foundational article we have written which offers a succinct overview of the current research and puts forward a conceptual framework for understanding how COVID-19 is associated with the onset of new sleep problems and the exacerbation of pre-existing sleep disorders. This framework will be incorporated into the overall TELECARE-ICU solution. What do you plan to do during the next reporting period to accomplish the goals?We will complete all six goals as decribed in the Interim Report which was submitted sbir@usda.gov In addition to the task for each of the goals, we will also be doing the following activities. File additional patents Publish at least 2 articles Pursue obtaining additional data sets. Continue our commercialization efforts with discussions with hospitals and companies.
